Policies

The Olde Forge’s Privacy Policy

Introduction

The OFCRC is committed to maintaining the confidentiality of the personal information of its clients, volunteers, employees, student placements, donors, members and any other stakeholders.

Personal information is given in trust and must remain confidential. It is important that personal information does not circulate outside of the OFCRC in an unauthorized manner, or within the OFCRC among staff for reasons other than appropriate consultations in the normal course of business.

The OFCRC collects personal information to provide quality community, social services and recreation programming as well as, during the course of various projects and activities.

Any individual who provides personal information can expect the information to be carefully protected and used only for the purpose(s) for which it was collected. Any other use of an individual’s personal information is subject to that individual’s consent.

The purpose of this policy is to ensure that personal information is appropriately collected, used and disclosed both within and outside of the OFCRC. The purpose is not to prohibit the discussion of personal information by staff for legitimate reasons.

Definition of Personal Information

Personal information is defined as any factual or subjective information, recorded or not, about an identifiable individual.

Examples are age, name, height, weight, medical records, information relating to physical or mental state (including family health history), name of health care provider, plans of service for provision of community services, ID numbers, income, ethnic origin, opinions, evaluations, comments, social status, disciplinary action, employee files, credit records, loan records, intentions (e.g., to change jobs).

Personal information does not include a job title, telephone number or address, anything that might appear on a business card, or can be found through publicly available information such as a telephone book.

Procedures

The OFCRC’s procedures are guided by 10 principles derived from the Canadian Standards Association’s ‘Model Code for the Protection of Personal Information’.

Both the Personal Health Information Protection Act (PHIPA) and the Personal Information Protection and Electronic Documents Act (PIPEDA) are based upon this code. These guiding principles are listed and defined as follows:

1. Accountability

The OFCRC is responsible for protecting and maintaining the confidentiality of all personal information under its control at all times. Staff, volunteers and students with access to records that contain personal information are expected to comply with the Privacy and Confidentiality policy. This responsibility includes ensuring that third parties with whom information is shared take steps to ensure the same levels of privacy as the OFCRC.

2. Purposes of Information Collection

Personal information will be gathered for specific purposes. The OFCRC is required to document the specific purpose(s) before it can collect and use personal information.

3. Obtain Consent

The valid and informed consent of the individual is required for the collection, use or disclosure of personal information, except as identified in the OFCRC’s Policies and Procedures.

4. Limit collection

The amount and type of information is limited to what is necessary for the identified purpose(s).

5. Limit use, disclosure and retention of personal information

The OFCRC will:
o Use or disclose personal information only for the purpose(s) for which it was collected, except with the informed consent of the individual or as required by law
o Keep personal information only as long as necessary to satisfy the purposes
o Keep personal information used to make a decision about a person for a reasonable time period. This should allow the person to obtain the information about the decision and pursue redress, if the person so desires
o Document any new purpose(s) for the use of personal information and obtain necessary consents.

6. Be Accurate

Personal information must be accurate, complete and as up to date as necessary for the purpose(s) for which is was collected.

7. Use Appropriate Safeguards

The OFCRC must protect personal information from theft and loss, as well as, unauthorized access, disclosure, copying or use.

8. Be Open

The OFCRC must have readily available specific information relating to the management of personal information and make it easy for interested individuals to contact the person accountable for OFCRC’s Privacy and Confidentiality Policy and Procedures.

9. Give Individuals Access

Upon request, individuals must be informed of the existence, use and disclosure of all of their personal information and given access to that information.

10. Challenging Compliance

Any individual (client, employee, etc.) can launch a challenge concerning compliance with the above principles to the Executive Director or to the Ontario Information & Privacy Commissioner.

Tribute Donations

Making a donation to honour a loved one or for a special occasion (birthdays, anniversaries, retirements etc.) is a meaningful way to show your support. If you would like for us to send a special note acknowledging the occasion, please indicate the intent of the donation and any other pertinent information.

Memorial Donations

A donation to honour the memory of a loved one can be a special gift to celebrate someone’s life. If you would like for us to send a special note acknowledging the gift, please indicate any instructions or pertinent information (such as a special message, name of recipient and mailing address) that you would like us to include.

When you sign up to be a monthly donor, your gift goes to support the many programs and services provided by the Olde Forge. Your ongoing support when becoming a monthly donor has an impact year-round helping to ensure the Olde Forge is able to support those that might need our services the most.

Volunteer Program

Volunteers are a cornerstone of almost all of Olde Forge programs. Volunteers generally provide the one-on-one contact and social interaction to our clients. Over all of our programs we are served by 175 dedicated, caring, competent volunteers.

 

Volunteer Application

Once we have received your application you will be contacted within two weeks, after which you will be asked to attend a personal interview with a Coordinator, apply for and provide a copy of a current Police Records Check, and participate in the mandatory training and orientation specific to your volunteer position.

How to become a volunteer

  1. Complete a volunteer application online or come into the Olde Forge and fill one out with our staff. Within 2 weeks a Coordinator contacts you to set up a meeting time to review your application and determine which areas of activities would be most compatible for your volunteering interests.
  2. Complete a Criminal Reference Check (CRC) that is appropriate for the position that you are interested in. After we provide you a volunteering letter there should be no or minimal cost for the Criminal Record Check.
  3. Read and sign required documents such as the Olde Forge’s confidentiality agreements, code of conduct, non-discrimination agreement, volunteer onboarding processes, and volunteer policies and procedures. Return the documents in either hard or soft copy to the Coordinator. The Coordinator saves the information in your volunteer electronic file.
  4. The Coordinator contacts your references with a series of standard questions to determine eligibility and fit.
  5. You will receive information about Ontario’s Accessibility for Ontarians with Disabilities Act (AODA) online training. Complete the training and send a copy of the certificate to the Coordinator.

Once you have completed the above steps you will be placed in a program and given information about orientation for your new volunteer position.

We really appreciate the transportation services as it offers true respite for caregivers supporting their elderly parents. It makes a giant difference in my mom’s life and supports us to be able to provide care for her in our home in a more sustainable way.

The Olde Forge’s volunteer program provides high quality volunteer experiences for staff, clients and volunteers.

The Olde Forge is enhancing our volunteer experience in 2024 and we remain committed to maintaining our connection and communication with our volunteers. Please talk with your Coordinator today if you have suggestions or feedback.

 

Yes. You will learn about the different dates, times available for particular volunteering activities. You are responsible for signing up for volunteer activities, timelines and teams that work with your schedule. Volunteers arrange their schedules by talking with their Program Coordinators.

The Olde Forge offers a variety of volunteering opportunities that require different lengths of time, different volunteering locations and commitments.

Contact your Program Coordinator if you need to change a shift. Your Program Coordinator will talk with you about the best way to get in touch with them during your program orientation.

Yes! After you become a volunteer The Olde Forge provides specific training for your assigned volunteer role.

Throughout the year, we may also have other training opportunities to offer such as First Aid, Food Safety training, Best Friends training and more! If you have ideas about other trainings that would be useful please share them with a Program or Volunteer coordinator.

You will most likely need a Police Record Check unless you are volunteering for a large group, public event. The level of the Police Record Check you need depends on the position you are volunteering for. Your Program or Volunteer Coordinator will determine the level of the Police Record Check needed and they will provide you with a copy of a volunteering letter that you include with your Police Record Check application. With the volunteering letter your Police Record Check should have no, or low cost. Visit Police Record Checks - Ottawa Police Service for more information.

The equipment needed depends on your volunteer position. Your Program Coordinator will be able to provide more detail during your orientation. If you have questions about equipment needs, please ask the Program or Volunteer Coordinator during your interview.

Expected time commitments vary based on the type of volunteer position you are interested in. Time commitments will be discussed during the interview between you and the Program or Volunteer coordinator. The Olde Forge is flexible and can accommodate vacations or leaves for volunteers. We only ask that you give your Program Coordinator as much notice as possible before you need to take time away from your volunteering.

You can volunteer starting at 14 years old. If you are between 14 and 16 and you would like to volunteer you will need to have adult supervision and support during your volunteer shift. Volunteers that are 16-18 years old will not require adult supervision to volunteer. Volunteers under 18 will require written parental or guardian consent.

Certain volunteer positions may have age eligibility requirements.

Transportation to Non-urgent Appointments

This program allows less mobile clients to maintain their ability to live independently and reduce their isolation by meeting important social and lifestyle needs involving sustenance, health and social contact. Clients are referred to other programs as needed if drivers notice changes in frailty or general health or social capacity.

Each day 10-18 people are served, with 7,000-8,000 drives offered each year.

Our 20 kind and caring volunteers ensure you arrive at your destination on time and safely home after.

How to register

Clients must be registered ahead and provide 7 days notice for requests. This program is open to all seniors and adults with disabilities who live in the Olde Forge catchment area.

Fees are charged for most types of rides, with extra costs covered by Forge budget.

Clients are responsible for payment of parking fees. Drives not cancelled are subject to charges.
